Social Worker -Children’s Disability Network Team
Medforce, representing an integrated care team dedicated to the well-being of Children, are actively searching for Senior and Staff Grade Social Workers to join a Multidisciplinary Team in a Children’s Disability Network Team in Co. Meath.
Responsibilities:
· Act as a direct point of contact for the local community in the designated service area, providing comprehensive social assessment and intervention to individuals and families.
· Collaborate with service users to identify their individual and collective needs, and co-develop early interventions and social action strategies to address those needs.
· Evaluate your own social work role and performance within your area of work, with a primary focus on assessing the outcomes of client-centered interventions.
· Offer emotional and practical support, along with information, to service users and their families, emphasizing independence, self-reliance, self-determination, and empowerment within their environment, families, and local groups.
· Assess cases where social determinants significantly impact health and social wellbeing, consulting and planning with service users and relevant teams/services, and coordinating appropriate support services for those in need.
· Adhere to current legislation, relevant policies, and protocols, placing particular emphasis on upholding the ethical principles of Social Work.
Candidate Requirements:
· Hold a recognized qualification as a Professionally Qualified Social Worker with CORU registration.
· Desirable experience in a similar setting.
· Must have permission to work in Ireland.
· Demonstrate a professional approach to work.
· Ability to work autonomously as well as under supervision.
· Possess a full, clean drivers license applicable in Ireland.
