Social Worker
Medforce Healthcare Recruitment is currently seeking a Professionally Qualified Social Worker (P.Q.S.W.) to join a Primary Care service in South Dublin.
This is an excellent opportunity for an experienced Social Worker to join a multidisciplinary community-based service supporting individuals and families across a diverse range of needs.
The successful candidate will work closely with colleagues across Primary Care and other community services, providing professional social work interventions and contributing to a person-centred approach to care.
Key Responsibilities of the Professionally Qualified Social Worker
- Provide professional Social Work assessment and intervention to service users and families.
- Manage an allocated caseload in accordance with professional standards and service requirements.
- Identify and assess social care needs and develop appropriate intervention plans.
- Work collaboratively with service users, families, carers, and other relevant professionals.
- Liaise with multidisciplinary teams and external agencies where appropriate.
- Advocate for service users and support access to appropriate community and social care services.
- Maintain accurate and up-to-date case records and documentation.
- Participate in multidisciplinary meetings and contribute to care planning.
- Work in accordance with relevant legislation, HSE policies, professional standards, and best practice.
- Contribute to service development and quality improvement initiatives.
Requirements for the Professionally Qualified Social Worker
- Recognised qualification in Social Work.
- CORU registration as a Social Worker is essential.
- Previous experience working as a Social Worker within a community, Primary Care, healthcare, or similar setting is desirable.
- Strong knowledge of professional Social Work practice and relevant legislation.
- Excellent communication, interpersonal, and organisational skills.
- Ability to manage a caseload effectively and work independently.
- Ability to work collaboratively as part of a multidisciplinary team.
- A full driver’s licence and access to a car may be required depending on service requirements.
