Social Worker
Medforce Healthcare Recruitment is partnering with Adult Primary Care Services in Laois to recruit a Professionally Qualified Social Worker. This is an excellent opportunity to join a collaborative multidisciplinary team dedicated to providing high-quality, person-centred support to adults living in the community.
As part of the Adult Primary Care team, you will work with individuals and families who require psychosocial support, advocacy, safeguarding guidance, and access to appropriate community-based resources. This role is ideal for a motivated and compassionate social worker seeking to make a meaningful impact across the Laois community.
Key Responsibilities of the Professionally Qualified Social Worker:
- Provide comprehensive social work assessments and interventions to adults presenting with a range of social, emotional, and practical support needs.
- Work as part of a multidisciplinary team including nursing, AHPs, psychology, and GP services.
- Advocate on behalf of service users to ensure access to community supports, entitlements, and appropriate services.
- Support adults experiencing challenges related to disability, chronic illness, mental health, safeguarding concerns, and social vulnerability.
- Develop person-centred care plans that promote autonomy, safety, and well-being.
- Maintain accurate records in line with CORU standards and HSE policies.
- Participate in service development and quality improvement initiatives.
Requirements for the Professionally Qualified Social Worker:
- Recognised Professional Qualification in Social Work (NQSW or equivalent).
- CORU registration as a Social Worker.
- Experience working with adult service users in a community, health, disability, or hospital setting is desirable.
- Knowledge of relevant legislation, safeguarding procedures, and social work frameworks.
- Strong interpersonal, assessment, crisis intervention, and advocacy skills.
- Full driving licence and access to a car (community travel required).
