Medforce Recruitment is delighted to recruit a Staff Grade Social Worker on behalf of a well-established Primary Care Team based in Mayo. This is an exciting opportunity to join a multidisciplinary team committed to delivering integrated, person-centred care in the community.
Key Responsibilities of the Social Worker:
- Provide psychosocial assessments and interventions for individuals and families in the community
- Support clients in addressing challenges such as illness, disability, mental health, family difficulties, and social isolation
- Work collaboratively with GPs, nurses, allied health professionals, and external agencies to coordinate care
- Advocate on behalf of clients and facilitate access to appropriate community services and supports
- Maintain accurate case records and participate in service planning, audits, and team meetings
Requirements for the Social Worker:
- A recognised qualification in Social Work
- CORU registration (essential)
- Experience in primary care, mental health, disability, or hospital settings is desirable
- Strong communication, organisational, and teamwork skills
- A full driver’s licence and access to own transport
