About the Role:
As a Client Care Manager, you will be responsible for assessing client needs, developing personalised care plans, and coordinating care services to enhance their well-being. You will work closely with clients, families, caregivers, and healthcare professionals to provide exceptional support and ensure care plans are delivered to the highest standards.
Key Responsibilities:
- Conduct comprehensive assessments of clients’ needs and create tailored care plans in collaboration with families and healthcare teams.
- Oversee and coordinate the implementation of care services, ensuring compliance with regulations.
- Regularly review and adjust care plans to meet evolving client needs.
- Provide guidance and training to caregivers, ensuring they have the necessary resources to deliver quality care.
- Act as the primary point of contact for clients and their families, addressing concerns in a compassionate and timely manner.
- Work closely with an interdisciplinary team to deliver holistic, person-centred care.
- Maintain accurate records of client assessments, interactions, and care plans.
What We’re Looking For:
- QQI Level 6 in Health Services Supervisory Management
- Experience in a healthcare or homecare setting
- Strong communication and interpersonal skills to build trust with clients and families
- Excellent organisational and problem-solving abilities
- Ability to work independently and collaboratively in a dynamic environment
- Proficiency in Microsoft Office and electronic health record systems
- Full driver’s license and access to a reliable vehicle (preferred)

Work Schedule:
- Monday to Friday
