Senior Occupational Therapist
Medforce are looking for a Senior Occupational therapist to work in Early Supported Discharge (ESD), ICPOP Team is part of the HSE’s national Integrated Care Programme for Older Persons. It is a specialist multidisciplinary team delivering rapid access to rehabilitation, reablement, and person-centred care in the community, supporting early discharge from acute hospitals and preventing re-admission.
Responsibilities of Senior Occupational Therapist
- Deliver specialist Occupational Therapy services to older adults with complex functional needs following hospital discharge.
- Conduct home and community-based assessments focusing on ADLs, equipment needs, mobility, cognition, and environmental safety.
- Collaborate with Physiotherapists, Nursing, Medical, Social Work, and other disciplines in delivering integrated care.
- Plan and coordinate early interventions that support recovery, reduce risks of readmission, and promote autonomy.
- Provide clinical supervision and support to staff-grade Occupational Therapists or students as required.
- Support service innovation, audit, quality improvement, and ICPOP-aligned development projects.
- Maintain accurate clinical documentation and contribute to service reporting and evaluation.
Requirements of Senior Occupational Therapist
- Hold a recognised qualification in Occupational Therapy approved by CORU.
- Be registered with CORU as an Occupational Therapist.
- Have a minimum of 3 years post-qualification experience, including substantial experience in hospital discharge planning, older person care, or rehabilitation.
- Demonstrate specialist clinical knowledge and reasoning skills in functional assessment, equipment prescription, and community-based rehabilitation.
- Proven ability to work as part of an interdisciplinary team.
- Full, clean driving licence and access to a car (essential for community visits).
Desirable Criteria
- Experience in early supported discharge, frailty management, or integrated care settings.
- Postgraduate training in manual handling, assistive technology, or cognitive rehabilitation.
