Senior Medical Social Worker
The Medical Social Worker will be a core member of the ICPOP multidisciplinary team, providing psychosocial assessment, discharge planning, and community integration support to older people transitioning from acute hospital care to home based in Naas. The role is key in supporting early discharge pathways while ensuring patients and their families receive holistic, person-centred care.
Key Responsibilities of Senior Medical Social Worker
- Conduct comprehensive psychosocial assessments for older patients referred to the ICPOP Early Supported Discharge team.
- Develop and implement individualized care and discharge plans in collaboration with patients, carers, and the multidisciplinary team.
- Provide emotional support and brief therapeutic interventions to patients and families dealing with adjustment to illness, disability, or care transitions.
- Facilitate access to community services, social supports, housing, home care packages, and safeguarding referrals as needed.
- Liaise with acute hospitals, community health teams, and voluntary agencies to ensure continuity of care.
- Advocate for patient rights and support person-centred decision-making, including advanced care planning and capacity assessments.
- Maintain accurate records and contribute to data collection, service evaluation, and audits.
- Participate in team meetings, reflective practice sessions, and staff training related to integrated care and gerontology.
Requirements of Senior Social Worker
- CORU registration as a Social Worker (essential).
- Minimum three years’ post-qualification experience, ideally in a hospital or older persons setting.
- Strong knowledge of relevant legislation: Assisted Decision-Making (Capacity) Act, adult safeguarding, mental health, and elder abuse protocols.
- Ability to manage a complex caseload and work both independently and collaboratively.
