Physiotherapist – Primary Care
The Physiotherapist will provide high-quality, evidence-based physiotherapy assessment, treatment, rehabilitation, and health promotion services within a primary care setting. This position is open to Staff (experienced) and Senior Physiotherapists.
The Physiotherapist will play a key role in early intervention, chronic disease management, falls prevention, musculoskeletal care, rehabilitation, and community-based support aimed at reducing hospital admissions and promoting self-management.
Responsibilities of Physiotherapist – Primary Care
- Conduct comprehensive physiotherapy assessments in clinic, home, community, or virtual settings.
- Develop and implement person-centred treatment and rehabilitation programmes.
- Assess and manage a broad range of conditions including:
- Musculoskeletal conditions
- Neurological conditions
- Respiratory conditions
- Age-related mobility issues
- Chronic pain and long-term conditions
- Provide interventions to improve mobility, strength, balance, endurance, posture, and functional independence.
- Deliver exercise prescription, rehabilitation programmes, and self-management education.
- Undertake falls risk assessments and falls prevention interventions.
- Assess mobility and recommend mobility aids and assistive devices where appropriate.
- Support rehabilitation following illness, injury, surgery, or hospital admission.
- Educate clients, carers, and families on treatment plans and prevention strategies.
- Maintain accurate, timely, and legally compliant clinical records.
- Participate in discharge planning and referral pathways as required.
- Promote physical activity, healthy lifestyles, and preventative healthcare.
- Deliver education programmes relating to mobility, posture, exercise, and injury prevention.
- Support chronic disease management and healthy ageing initiatives.
- Identify risks affecting physical health, safety, and independence.
Requirements of Physiotherapist – Primary Care
- Degree or recognised qualification in Physiotherapy.
- Current registration with CORU
- Experience working as a Physiotherapist in primary care, community, rehabilitation, acute, or related healthcare settings.
- Experience assessing and treating a varied caseload independently.
- Experience working within multidisciplinary teams.
- Experience delivering rehabilitation and exercise-based interventions.
