About: Our client is a dedicated Disability Service Team committed to providing high-quality, person-centered care. They collaborate with stakeholders to enhance service delivery and meet the evolving needs of individuals with disabilities.
Roles and Responsibilities:
- Work as part of the Disability Service Team to develop and manage service structures.
- Build relationships with relevant stakeholders to maximize service provision.
- Inform service development and prioritize needs where gaps are identified.
- Develop individualized service options in consultation with clients, families, and relevant professionals.
- Present costed business cases for service provision as needed.
- Coordinate and attend multidisciplinary case management reviews.
- Foster links with statutory, voluntary, and private service providers.
- Deputize for the Disability Manager when required.
- Serve as Assessment/Liaison Officer (LO) under the Disability Act 2005 when needed.
- Maintain up-to-date written and electronic records in accordance with professional standards.
- Efficiently manage budgets, resources, and prepare financial reports as necessary.
- Participate in research, development, and service evaluations.
Qualifications and Experience:
- Significant experience in Disability Services and case management.
- Proven experience working as part of a multidisciplinary team in Disability Services.
- Experience in service coordination, placement, and discharge liaison.
Skills, Competencies, and Knowledge:
- In-depth knowledge of the Disability Act 2005, ADM (2015), Health Act 2004 & 2007, and HIQA regulations.
- Understanding of health service structures, social care services, and disability sector services.
- Proven ability to implement change, evaluate information, and solve complex problems.
- Strong decision-making skills with the ability to explain decisions clearly.
- Excellent communication, interpersonal, and report-writing skills.
- Strong planning, organizational, and leadership abilities.
- Commitment to delivering high-quality service and promoting continuous improvement.
