Social Worker
The Social Worker will provide professional social work services to individuals, families, and communities within North Tipperary Primary Care. The role involves assessment, intervention, advocacy, and collaboration with multidisciplinary teams to support service users in improving their overall well-being. We will accept either a Staff Grade Social Worker or a Senior.
Key Responsibilities of Social Worker
- Provide social work assessments and interventions for individuals and families experiencing psychosocial difficulties.
- Work with clients experiencing challenges such as mental health issues, domestic violence, housing insecurity, disability, financial difficulties, and bereavement.
- Advocate for service users and provide guidance on social welfare entitlements, housing applications, and community resources.
- Support service users in navigating healthcare, legal, and social care systems.
- Develop and implement care plans in collaboration with clients, families, and the wider multidisciplinary team.
- Work closely with GPs, public health nurses, physiotherapists, occupational therapists, and other allied health professionals.
- Attend and contribute to case conferences, team meetings, and service planning discussions.
- Facilitate referrals to other healthcare, social care, and community support services as appropriate.
- Provide early intervention support for families facing difficulties, ensuring the well-being of children and vulnerable adults.
- Offer crisis intervention and risk assessments in complex cases.
- Maintain accurate and confidential records in accordance with legal and professional guidelines.
- Adhere to the professional standards set by CORU and the HSE Social Work Code of Practice.
- Participate in continuous professional development, training, and supervision sessions.
- Comply with health and safety regulations and contribute to risk management strategies within the service.
Requirements for Social Worker
- A professional social work qualification recognised by CORU.
- Registration with CORU as a Social Worker (or eligibility for registration).
- Previous experience working in a health or social care setting, preferably in primary care, mental health, or family support services. Placement experience is fine.
- Ability to work independently while also collaborating within a multidisciplinary team.
- A full clean driving licence and access to a vehicle for community visits.
