Job Description:
As a Social Worker in our Mental Health team, you will work with people who are experiencing a wide range of mental health conditions. You will play a vital role in assessing, planning, and delivering care and support to improve their well-being and independence. Working closely with healthcare professionals, families, and community resources, you will ensure that our service users receive the holistic care they need.
Key Responsibilities:
- Conduct assessments of individuals’ mental health needs
- Develop, implement, and review care plans
- Provide emotional support, counseling, and advocacy
- Collaborate with healthcare professionals, support services, and families
- Ensure service users have access to the appropriate resources and services
- Safeguard vulnerable adults and maintain up-to-date records
- Support service users in achieving their personal recovery goals
Requirements:
- Degree in Social Work and registration with CORU
- Experience working with people in mental health services
- Strong understanding of mental health issues, recovery approaches, and social care systems
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Empathy, patience, and a commitment to improving the lives of others
