Social Care Worker CNDT
Role Purpose
The Social Care Worker will be a key member of the Children’s Disability Network Team (CDNT), supporting children and young people (0–18 years) with complex developmental needs and their families. The role focuses on promoting child-centred, family-focused, and strengths-based practice in line with Progressing Disability Services (PDS) and HSE standards. This role is based in
Key Responsibilities

Work collaboratively as part of a multidisciplinary CDNT, including psychology, OT, SLT, physiotherapy, social work, and nursing.

Support children and families through individualised care planning, goal setting, and intervention delivery.

Provide practical, emotional, and behavioural support to children with physical, sensory, intellectual, and/or neurodevelopmental disabilities.

Facilitate group work, social skills programmes, and developmental interventions as required.

Support families to navigate services, advocate for their child’s needs, and access community resources.

Contribute to assessment, intervention, and review processes, ensuring outcomes are measurable and child-centred.

Maintain accurate, timely, and confidential case records and reports in line with organisational and GDPR requirements.

Uphold Children First, safeguarding, and duty of care responsibilities at all times.

Promote inclusion, independence, and participation for children within their family, school, and community settings.

Participate in team meetings, supervision, training, and continuous professional development.
Essential Criteria

Degree in Social Care or equivalent (Level 7/8) recognised by CORU.

Must be registered with CORU as a Social Care Worker at the time of application.

Experience working with children with disabilities and/or complex needs (CDNT, disability, or family support setting desirable).

Strong understanding of child development, family-centred practice, and disability models.

Excellent communication, interpersonal, and teamwork skills.

Ability to work independently and as part of a multidisciplinary team.

Knowledge of Children First, safeguarding, and relevant legislation.

Full clean driving licence and access to a car (desirable, depending on service delivery).
Desirable Criteria
-
Previous experience within a CDNT or disability service.

Experience delivering group-based interventions or parent support programmes.

Knowledge of Progressing Disability Services (PDS).
-
Experience working with neurodiversity (ASD, ADHD, developmental delay).

Salary & Benefits

Salary: €40,351 – €56,650 per annum (HSE consolidated Social Care Worker pay scale, dependent on experience).
